We make it easy to refer your patients! There are several ways to send a referral to Mountain View Eye Center. Choose whichever method works best for your practice.

- Call our Co-management Coordinator at 801-773-2233, extension 1019.
- Email our Co-management Coordinator at referrals@utaheyedoc.com.
- Download a referral form, complete the form, and fax to 385-383-7033.
- Fill out our referral card and give to the patient; have the patient call us to schedule. If you need more referral cards, please let us know by phone/email listed above.
