What to Bring toYour Eye Appointment

  • Your current glasses and/or contact lens boxes
  • A list of your current medications, including dosage (prescription and non-prescription)
  • Eye drops you are currently using, if applicable
  • A current driver’s license or state ID
  • Medical and Vision Insurance Cards
  • Physician referral if required by your insurance
  • Sunglasses since many exams require dilation. We have disposable shades if you do not have sunglasses
  • Flex Spending or Health Savings card, credit card, cash, or check to pay your co-pay and any account balance you may have
  • You may print and complete the new patient forms listed below and bring them to your appointment, or we can provide these forms for you to complete upon your arrival

Accepted Insurances

  • We accept most major health insurance plans. When scheduling your first appointment, please have your insurance card available to provide details to our scheduler.
  • We participate with several vision plans including VSP, EyeMed, and Opticare
  • We are considered specialists and some insurances require referrals from your primary care physician (PCP).If your insurance requires referrals, it is your responsibility to acquire them prior to your scheduled appointment.
